  • Project Request Form

    Technology and Educational Support Services (TESS) utilizes a project prioritization process to solicit input from TESS Executive Committee and establish project priorities for requested projects. The purpose of this form is to provide TESS enough basic information to determine the scope and resource demands of your request.

    IMPORTANT: Filling out this form, or the creation of the project charter, does not mean the project has been accepted or approved. A Completed "Project Request Form" must be recieved by the deadline posted in the Project Prioritization Schedule.

  • Purchasing 2000 Account Set-Up (hosted at the SBCCD Wiki)

    This form is mainly for managers and their designated staff that need access to Financial 2000.

  • Student E-mail - Club Request Form

    This application is for those student clubs that want an email address for the club. Please fill it out as completely as possible. Mail this form to "District Computing Services, Attn: Administrative Assistant". Please expect 1 to 3 days for an initial reponse to your request.

  • Training Request Form

    The purpose of this form is to provide TESS enough basic information to determine the necessary scope and resource demands of your request. This form:

    • is completed by the person(s) requesting and/or coordinating a training session.
    • is to be submitted to the Director of Administrative Application Systems in District Computing Services, for approval and resource assignment.
    • must be signed and received by e-mail, fax or inter-campus mail, at least, 10 business days prior to the training date requested.

    IMPORTANT: By submitting this request, it does not mean that the training and/or resources will be available to approve it. 

  • User Application (Formerly known as the "New User Application")

    You need to fill out a user application if you:

    • Are a newly hired staff or faculty member
    • A returning employee who was previously terminated (stopped employment as compared to on extended leave)
    • Are transferring to a different position
  • Web Editor Access Application

    This application is for those faculty/staff/students who need to apply for access to update a faculty/department/club website. Please fill it out as completely as possible. Mail this form to "District Computing Services, Attn: Web Services". Please expect 1 to 3 days for an initial reponse to your request.
